Skills Needed:	REQUIREMENTS EDUCATION: Current Georgia license as RN. EXPERIENCE: Case management experience helpful. Home Health and/or Hospice experience as well as Insurance background preferred. SKILLS: Excellent oral and written communication skills. Must possess good documentation skills.
Job Description:	Responsible for care management of patients. Responsible for telephonic coordination of patient’s healthcare needs between providers and commercial insurance. Must be organized and able to manage time effectively. On Call is required.
