EDUCATION: Graduate of an APTA approved school and holds valid state license.

EXPERIENCE: 3 years of homecare experience preferred.

SKILLS: Proven evaluation and treatment skills are required. Has the ability to establish and maintain strong relationships. Has the ability to develop and implement new programs and techniques. Effective verbal, written and interpersonal communication skills are essential. Basic computer knowledge is required

Evaluates and treats all patients referred by a licensed physician. Maintains effective communications with the facility staff, patients, families and management. Timely and appropriately documents patient progress. Orders or assists in obtaining equipment or supplies to meet therapeutic goals. Travels and works weekends as required.
