UniHealth SOURCE (Service Options Using Resources in Community Environments) offers easy access to Georgia’s Medicaid SOURCE program, which is designed to provide elderly, disabled or low-income patients with a range of long-term health care services at home in the community.

Our nurse care managers aid and monitor health care services of patients at little or no expense. Services provided through UniHealth SOURCE include home health, personal support, homemaking, home-delivered meals, emergency response, caregiver respite, transportation, errands, adult daycare, personal home care and outpatient therapy.

UniHealth SOURCE can admit and monitor patients 24 hours a day, seven days a week.
