By coordinating with the physician and family, the staff is able to develop an individual plan of care. Services are designed with one goal in mind….. to assist patients in reaching their optimum level of function while in the comfort of their own residence.
Nurses come to the place of residence or hospital to perform an assessment according to a physician’s order. If appropriate, patients are admitted to the program. A plan of care is then designed to specifically meet the needs of the individual patient and according to the orders of the physician.
United Home Care provides home health services for Medicare, Medicaid, commercial and private insurance patients in many counties across Georgia. Services are available to our patients 24 hours a day, 7 days a week.
